If you are unable to download and completed the admission paperwork prior to your scheduled appointment, please arrive 30 minutes before your appointment time to complete the necessary paperwork
What you need to bring to your first visit:
1. Patient or Parent/Guardian (if < 18 y/o) Photo ID
2. Doctor’s order for physical therapy, if referred by a physician.
3. Completed Admission Packet.
4. Any insurance cards you would like your services filed on.
